I ordered lifeline phone service along with my application on 5/09/12. It has been over two months and I have yet to receive proper notice of actions taken on two (2) application I was asked to submit. I have been billed along with a cut off notice for $84.99. I requested "reasonable accommodation" in rules and policy relatiog to being billed a t normal rates for services I did not request as normal and be given an adjusted bill. My name is Micheal Hall. My Phone #(Personal Information Removed). I request that this be resolve as a "reasonable accommodation" to rules and policy prior to shutoff.
